I give my consent for a Natalie Diaz, CLC to work with me and my baby for my specified breastfeeding challenges/concerns.
 
I understand that a visit with Natalie may involve: viewing and touching of my breasts and/or nipples for the purpose of assessment and inspection, observation of my latch techniques and will offer advice and suggestions to correct/enhance the latch and/or positioning of my baby.
 
I understand that my visit may include the inspection of any lactation equipment such as my breast pump or supplies.
 
I understand that Natalie Diaz will not perform any medical procedures nor will she go against any recommendation my healthcare provider has offered. She will, instead, assist with preparing a list of questions and concerns I may present to my pediatrician for further clarification.
 
She will not provide me or my baby “treatment” via oral medications (prescription or over-the-counter) but can point me toward what I might need so that I can discuss treatment options with my pediatrician.

I understand and agree to inform my Pediatrician about any supplements (galactagogues) that are commonly used to “increase” or “boost” breastmilk production BEFORE I use them.

I understand that I am responsible for informing Natalie Diaz of changes I feel are necessary for her to know while she is assisting me with Lactation Services and during follow-up services.

I give my consent for Natalie Diaz to release any information she has collected during our visit about myself or my baby to our healthcare provider, pediatrician, referring providers, and/or other Lactation Specialists should it be deemed necessary.

I understand that for this lactation visit and any follow-ups I may have, Natalie Diaz will protect the privacy of my personal health information as required by law and the Health Insurance Portability and Accountability Act (HIPPA).
 
I understand that payment is due at the time services are rendered. Natalie Diaz does not guarantee that your health insurance company will reimburse any costs for her services.

Health Insurance Reimbursement

  • All fees are due before or during your visit. 
  • Most clients get full cost reimbursement for an In-Home/Office/Virtual Lactation visits with Natalie Diaz through their health insurance company. If you are planning on submitting a claim, please request a superbill no later than 30 days after your visit. It will contain all the information your insurance company will require.

What's Included:

During the 2-hour breastfeeding strategy session with Natalie Diaz, you will have the opportunity to discuss and address any challenges or concerns you may have related to breastfeeding.

The session will be personalized to meet your specific needs and goals.

Here is an outline of how the session may be structured:

 Introduction and Assessment: 
- Natalie will introduce herself and create a comfortable environment for you and your baby.
- She will ask about your breastfeeding experience so far, any difficulties you've faced, and your goals for the session.
- Natalie will evaluate your baby's latch, positioning, and overall breastfeeding technique.

Education and Guidance:
- Natalie will educate you on the basics of breastfeeding, including the benefits, proper latch, positioning, and milk supply.
- She will address any misconceptions or myths related to breastfeeding.
- Natalie will provide strategies and techniques to improve latching and positioning, ensuring optimal milk transfer.

Troubleshooting:
- You will have the opportunity to discuss any specific breastfeeding challenges you've encountered.
- Natalie will analyze the underlying causes of these challenges and suggest targeted solutions.
- She will provide tips for managing common breastfeeding issues such as nipple soreness, engorgement, or low milk supply.

Pumping and Milk Storage:
- If applicable, Natalie will guide you through the process of pumping breast milk.
- She will explain proper storage techniques and provide recommendations for maintaining milk supply while pumping

Establishing a Routine and Support:
- Natalie will help you develop a personalized breastfeeding routine that fits your lifestyle and preferences.
- She will discuss strategies for managing breastfeeding in public or returning to work while maintaining breastfeeding goals.
- Natalie will provide information on local breastfeeding support groups or resources you can turn to if you need further assistance.

Throughout the session, Natalie will also offer breastfeeding tips and answer any additional questions you may have. She will ensure that you feel supported and empowered to continue your breastfeeding journey successfully.
